SMC Manhattan Health Center – Lotus Health and Wellness

 

 

PHYSICIAN - Part-time

 

SUMMARY:  SMC Manhattan Health Center – Lotus Health and Wellness is currently offering a part-time position for a Family Practitioner to provide primary medical care to patients including diagnosis, treatment, consultation and follow-up in a strictly outpatient setting.  Providers will be expected to provide high quality of care for all patients on their panel, and have a desire to be an integral part of a developing team of clinicians dedicated to changing health care delivery in this area. Providers will also be expected to participate in QA/QI initiatives.

 

QUALIFICATIONS:  Graduate from an Accredited Medical School; Valid NYS Medical License required; DEA Registration; Board Certified in Family Practice or Internal Medicine; BCLS or ACLS/PALS Certifications; Ability to attain privileges in community hospitals; Proficient in electronic health record (EHR); 2-3 years clinical experience; Excellent time management skills a must; bilingual (Spanish) preferred. 

 

FAMILY NURSE PRACTITIONER – Full-time

 

SUMMARY:  Provide continuous comprehensive care through disease management, health promotion, health education and preventive health services; assess, diagnose, and treat patients from childhood to adult on a long-term ongoing basis; perform medical examinations and prescribe medications.

 

QUALIFICATIONS:  Master’s Degree in Nursing with a specialization in family nurse practitioner from an accredited college or university; Valid NYS License in Advance Practice Registered Nurse; current DEA registration; current CPR Certification or ACLS; Board Certified ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners; Proficient in electronic health record (EHR); 2-3 years nursing experience in specialized care in a family health clinic; time management skills a must; bilingual (Spanish) a plus.

 

NURSE MANAGER – Full-time

 

SUMMARY:  Provide general nursing care to patients; assist provider with examinations and procedures; assessments, provide physical and psycho-social support to patients; provide training, orientation and education to staff; manage the interdisciplinary care team; participate in QA and clinical committees.

 

QUALIFICATIONS:  Bachelor’s Degree in Nursing from an accredited college or university, Master’s Degree preferred; Valid NYS Registered Nurse License; knowledge of nursing care in accordance with regulations and practices established by the NYS Board of Nursing; 2-3 years of primary care experience; Proficient in electronic health record (EHR); BCLS certification;  leadership and administrative skills required; bilingual (Spanish) a plus.

 

 

 

 

 

LICENSED CLINICAL SOCIAL WORKER – Part-time

 

SUMMARY:  Provide assessments, mental and substance use evaluations, diagnosis and treatment planning; social services to patients and their families, individual and group sessions; participate in QA and other clinical committees as appropriate.

 

QUALIFICATIONS:   Master’s Degree in Social Work from an accredited college or university; Valid NYS License – LCSW; 1-2 years of acute care experience; Proficient in electronic health record (EHR); Strong interpersonal and communication skills required; bilingual (Spanish) preferred.

 

REGISTERED DIETICIAN – Part-time

 

SUMMARY:  Provide assessments, care planning, medical nutritional therapy; participant of interdisciplinary care team, QA and clinical committees as appropriate.

 

QUALIFICATIONS:  Master’s Degree in Nutrition from an accredited college or university; Valid NYS License-Registered Dietitian; 2 years of acute care experience Proficient in electronic health record (EHR); Strong interpersonal and communication skills required; bilingual (Spanish) a plus.
