2009 vs. 2010

For all clients enrolled in the adult day treatment program during the 12 month review period, the proportion of clients who had CD4 and viral load testing every four months in each of the four month periods in which the client was eligible.

For all clients enrolled in the adult day treatment program for all 12 months of the review period, the proportion of clients who maintained stable clinical status during the review period based on their viral load laboratory values. A client is considered clinically stable if his/her viral load is undetectable, under 400 for each viral load documented during the 12 month period.

For all clients enrolled in the adult day treatment program for a minimum of 30 days in the period of review and on ARV therapy, the proportion of clients whose adherence to ARV therapy has been quantified (total number of doses taken divided by the total doses prescribed over the past three days), discussed, and documented on a monthly basis in each of the four months prior to the client’s last visit AND of those clients for whom an adherence problem/barrier has been identified, the proportion of clients for whom this issue is documented on the care plan.

For all clients admitted to the adult day treatment program in the year of review and enrolled for a minimum of 30 days, the proportion of clients who had an initial mental health care assessment performed that included all of the following: cognitive functioning, including mental status; depression screening; anxiety screening; sleeping habits; domestic violence screening; PTSD screening; psychiatric history, including current and past treatment; and current and past psychiatric medication history.

For all clients enrolled in the adult day treatment program for four consecutive months in the year of review, the proportion of clients who had a mental health care reassessment performed in each quarter of the review period that included all of the following: depression screening, anxiety screening, domestic violence screening, and PTSD screening.

For all clients enrolled in the adult day treatment program for a minimum of 30 days in the year of review and for whom an unmet mental health need was identified, the proportion of clients who had a referral made for an identified need (referrals may be to either an in-program or out-of-program provider.) AND of those clients who had a referral made for an identified need, the proportion of clients whose appointment with a mental health provider occurred within 30 days of the referral.

For all clients who were prescribed psychotropic medications for at least one quarter of the review period, the proportion of clients whose adherence to psychotropic medications has been assessed, either quantitatively or qualitatively, and discussed and documented in all eligible quarters AND of those clients for whom an adherence problem/barrier has been identified, the proportion of clients for whom this issue is documented on the care plan in each eligible quarter.

For all clients with a history of substance and/or alcohol use within the last three years, the proportion of clients for whom substance and/or alcohol use has been assessed and documented in all eligible quarters within the review period AND of those clients for whom a substance and/or alcohol use issue has been identified, the proportion of clients for whom an intervention has been delivered and documented.

For all clients enrolled in the adult day treatment program for four consecutive months in the year of review, the proportion of clients for whom high-risk sexual behavior has been assessed and documented in all eligible quarters of the review period AND of those clients for whom high-risk sexual behavior has been identified, the proportion of clients for whom an intervention has been delivered and documented in all eligible quarters within the review period.

For all clients enrolled in the adult day treatment program for four consecutive months in the year of review, the proportion of clients for whom tobacco use has been assessed and documented in all eligible quarters within the review period AND of those clients for whom tobacco use has been identified, the proportion of clients for whom an intervention has been delivered and documented in all eligible quarters within the review period.

For all clients enrolled in the adult day treatment program for four consecutive months in the year of review, the proportion of clients with stable housing. A client is considered with stable housing if: the client is in housing that is medically appropriate, and the client is not facing eviction.

For all clients enrolled in the adult day treatment program for a minimum of 30 days and for whom a housing need was identified, the proportion of clients who had a referral made for an identified housing need AND of those clients who had a referral made for an identified housing need, the proportion of clients whose appointment with a housing provider occurred within 30 days of the referral.
